This complex care package was based in Northumberland; the service user was a 60-year-old gentleman who received an Acquired Brain Injury (ABI) when incarcerated. The service user had a history of sexual assault, which resulted in extremist views and sexualised behaviour post-ABI. The service user was evicted from a care home due to sexual misconduct and after assaulting a police officer, was released into the community with a 2:1 staffing team from Standby.

Introduction
The service user was independent with personal care but needed support around anti-social behaviours and emotional responses. This presented a uniquely difficult challenge, as we had to build a staffing team that was all male, to safeguard against any risk behaviours toward female staff.
Our bookings and compliance team had to work together to ensure staff members were aware of procedures and risk when out in the community and that a key information document was supplied by the case manager of the individual. Support focused on community integration, risk reduction strategies and medication administration. Our bookings team had to recruit quality, reliable and experienced staff into the team so that the staff, service user and community were safeguarded.
Care Package Overview
The service user demonstrated a variety of risk behaviours, including sexual advances, emotional outbursts, and signs of aggression. Our staff team kept open communication with the bookings team to feedback any issues and happenings. Through this feedback, an order was placed to restrict internet access to the service user. This reduced risk behaviours online, but exasperated behaviours in the community. To overcome this, specialist training was devised for the staff team around working with ABI as well as physical intervention techniques, with positive results.
When the package first began, the service user did not engage with staff or the community in a positive way, displaying many risk behaviours. Staff worked tirelessly to prevent any potential issues and the risk of re-offending.
Standby have now supported this service user for eight months, with a regular rota, staff members have built a rapport with the service user and developed strategies to keep the service user, staff, and the community safe.
Going Forward
The staff team are continuing to support the service user and is working to reintegrate them into the community. Staff are searching for day centres and activities that may be suitable for the gentleman as well as workshops to help with their viewpoints.

The service user was 17 years old when the package began and is now 18 years old. The service user has a diagnosis of Frontal Lobe Syndrome, which affects their executive functioning. This presents as symptoms of limited working memory, weak understanding of complex language and difficulties with emotional regulation.
